Subscriber Agreement
CAREGURU is engaged in the business of facilitating access to medical and para-medical healthcare products and services. This Agreement sets out the terms and conditions pertaining to the provision and use of such healthcare products and services. By ticking against the "I Agree to the Terms and Conditions" check-box and completing the registration process, you confirm that you agree to be bound by all the terms and conditions of this Agreement as well as the Beneficiary Agreement, which shall be read as a part and parcel of this Agreement.

1.
Network
1.1
All healthcare services and products are provided through a network of empanelled general practitioners, specialists, hospitals, clinics, diagnostic centers, etc. CAREGURU assures the Subscriber that all such service providers have been selected with utmost care and after due review of their expertise and skills.
1.2
There is no bar on the Subscriber and/or the Beneficiaries or any of them from utilizing the services of hospitals, general practitioners, specialists, hospitals, clinics, diagnostic centers, etc., outside the network of such services provided by CAREGURU. However, CAREGURU shall not be obliged to provide any support services to the Beneficiary(s) and/or information to the Subscriber in respect of such services outside the CAREGURU network.
2.
Identity Cards
2.1
Upon completion of the registration process and upon execution of the Beneficiary Agreement, each of the Beneficiaries will be provided with an Identity Card (ID Card).
2.2
The Beneficiary/(ies) should retain the ID Card carefully and produce the same on demand by CAREGURU, its representatives, and/or any of the network service providers in order to be entitled to receive specific services subscribed for.
2.3
In the event of the Beneficiary's failure to produce the ID Card on such demand, such Beneficiary may not be entitled to the privileges of the CAREGURU services subscribed for and the members of the network may offer their services on their usual terms. The Subscriber may however approach CAREGURU for replacement of lost/misplaced card, which would be done at a nominal fee.
3.
Payments
3.1
All services and/or products are being provided in the manner and on payment of certain specific charges as described on the website.
3.2
Subscriber acknowledges that the price and payment methods are available on the website and that he / she is fully aware of it at the time of subscription. Subscriber is also aware that CAREGURU may prospectively change the specified prices and the payment methods from time to time.
3.3
Any charges incurred for hospitalization, if at any time, before/during/after health checks covered under the CAREGURU services, are incurred over and above the specific charges mentioned on the website, and the Subscriber and/or the Beneficiaries or any of them shall be liable for making provision for payment of such amount over and above the said charges. CAREGURU shall not be liable for any delay in treatment and/or for any other consequential loss, harm and/or injury to the Beneficiary(s), due to the delay in the receipt of the payments.
4.
Services Guide
4.1
All Beneficiaries will be provided with written instruction relating to the availability of services and products, and the manner in which the same should be utilized.
4.2
CAREGURU and/or the network members, such as general practitioners, hospitals, clinics, specialists, etc., or any of them shall not be liable for any harm, loss, injury, etc., that may be suffered by the Beneficiary(s) as a result of non-compliance with the instructions.
5.
Availability Of Services
5.1
In the provision of and access to all the healthcare services, CAREGURU will make its best efforts to ensure that all the members of the network are available, at their specified times, to the Beneficiary(s). For example, CAREGURU will endeavor to fix an appointment with the required member of the network at a time convenient to the Beneficiary(s). However, if the required member is not available at the desired time, CAREGURU will make a best faith effort to fix an appointment at a time convenient to the Beneficiary(s).
5.2
However, CAREGURU will not be held responsible for any harm, loss, injury, etc., that may be suffered by the Beneficiary(s) due to non-availability of network members.
6.
Emergency Requirements
6.1
CAREGURU’s services are limited to diagnostic and preventive healthcare services. Beneficiaries, during the term of their CAREGURU membership, are eligible to seek emergency, inpatient, and outpatient medical services from the member hospitals in the CAREGURU network, directly. Beneficiaries may be eligible for special discounts with the member hospitals.
6.2
In the event of an emergency, the Beneficiary is also free to seek medical assistance, directly, from personnel or establishments outside the CAREGURU network.
7.
Physician Calls
7.1
The Subscriber is entitled to two phone calls with a physician per year for each beneficiary.
7.2
The Content of physician’s phone calls are informational in nature is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Users should always seek the advice of their physician or other qualified health provider with any questions they may have regarding a medical condition without delay.
7.3
Any information so rendered is purely for the sake of information and does not constitute medical advice. Please consult a specialist before acting on any of the information. CAREGURU does not accept any liability or responsibility for any damage caused by acting on such information. Also, CAREGURU will take the best efforts to ensure that the expert can meet the timings but assumes no responsibilities for delays caused, if any, outside its control.
7.4
The information provided through CAREGURU is not intended to replace medical or surgical diagnosis, treatment, and management discussed by the user's Doctor. The user is urged not to disregard medical advice or delays in seeking medical attention because of or based on any information that he/she may have read or received on the CAREGURU site.
8.
Information To The Subscriber
8.1
CAREGURU will put up all information relating to the medical history, records, etc., of the Beneficiary on the website.
8.2
All such information will be up-dated periodically to keep the Subscriber informed of the progress concerning the medical condition of the Beneficiary.
8.3
However, it shall be the responsibility of the Subscriber to access the website on a regular basis to secure the information put up by CAREGURU.
9.
Authority to Disclose Medical Information
9.1
The Subscriber hereby authorizes CAREGURU and the members of its network, and further, assures CAREGURU that he/she will secure the consent of the Beneficiary(s) to authorize CAREGURU and the members of its network, to collect and disclose all information and/or copies of records relating to the diagnostic test results, medical history, records, condition of health, treatment, costs, etc., of the Beneficiary(s) to the Subscriber.
9.2
The Subscriber hereby indemnifies CAREGURU and the members of its network from any claim, loss, liability, etc., that may be made by the Beneficiary(s) against CAREGURU and/or any of the members of its network due to the disclosure of such information to the Subscriber.
10.
Medical and Para-Medical Support Services
10.1
As and when required by the Subscriber and/or the Beneficiary(s), CAREGURU may assist the Beneficiary in identifying medical and para-medical support services such as hospitals, doctors, nurses, physiotherapists, etc.
10.2
However, CAREGURU's role will be restricted to, merely, identifying such qualified medical and para-medical service providers and to communicate their contact details to the Beneficiary(s).
10.3
CAREGURU shall not, in any manner whatsoever, be responsible for the selection, quality, accuracy of services, costs, etc., of such medical or para-medical service providers.
10.4
Further, CAREGURU shall not be responsible for any loss, harm and/or injury that may be suffered by the Subscriber and/or the Beneficiary(s) due to the use of the services of such medical or para-medical service providers.
11.
Refunds and Order Transfer Policy
11.1
An administrative cost of US$25 will be deducted / charged on all cancellations made within two weeks after the time of placing the Order. No refund shall be issued two weeks after the time of placing the order.
11.2
CAREGURU shall make a maximum of six attempts to coordinate health check visits for each beneficiary. After the above-mentioned six attempts, the beneficiary would need to contact CAREGURU to schedule a health check.
11.3
The beneficiary has a maximum of one year from the date the order is placed to avail CAREGURU services under the plan. If the beneficiary does not avail the service within one year, the service shall be forfeited with no refund.
12.
Transportation
12.1
Transportation service is provided only within a 25 km radius of the partner hospital/facility chosen for health checks.
12.2
The transportation service will include two (2) pick up and a drop-off to the same location for one paid-up subscription.
12.3
In the event the subscriber purchases transportation service for the beneficiary and if the beneficiary does not avail the transport provided by CAREGURU to visit the partner hospital/facility in the Metropolitan areas, for reasons best known to him/her, claims for refund of transport component will not be entertained.
